The Villa, a 5 STAR senior care community of LSMNJ, located in desirable Florham Park, is currently seeking a Social Worker.

General Responsibilities

The Social Worker is responsible for advocating for residents in all aspects of their life by developing a one-to-one relationship with residents and responsible parties. Also, coordinates these efforts with interdisciplinary team.

Essential Duties

  1. Supports the mission and values of Lutheran Social Ministries of New Jersey, which is: Through the power of the Holy Spirit, and in response to God’s love as revealed in the Gospel, the mission of the Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, are in need, or have limited choices.

  2. Completes timely and accurate social work assessments per policy; making appropriate referrals as needed for Hospice, and/or special needs.

  3. Serves as resident/client advocate. Resolves resident concerns by taking prompt, active steps. Works at resolution of a problem until the resident is satisfied.

  4. Provides education of resident rights to all residents, staff, and family.

  5. Implements and coordinates programs for residents and families which enhance the quality of resident life.

  6. Appropriately assists residents in problem solving, adjustment, interpersonal issues, family concerns, etc.

  7. Completes Multiple Data Set (sections: AB, AC, E, F) as needed for every resident.

  8. Completes timely discharge plans to include the coordination and arrangements of goods/services for home care.

  9. Coordinates room changes along with Admissions and Nursing Department.

  10. Provides appropriate information regarding advanced directives.

  11. Facilitates care conferences with families/POA’s and interdisciplinary team.

  12. Actively demonstrates participation in resident care plan interventions.

  13. Ensures every resident is properly clothed.

  14. Oversees and coordinates all lost and found initiatives and documentation.

  15. Ensures all documentation and services comply with State and Federal regulations as well as JCAHO accreditations.

  16. Processes rep payee requests and provides Medicaid information, requesting PAS’s as needed.

  17. Attends Utilization Review meetings, communicates payor source changes with residents/POA’S, and processes communication form to all necessary parties.

  18. Participates in Quality Assurance program, performing audits on SS areas.

  19. Markets facility as needed by facility.

  20. Accountable for alerting SS Director and/or Administrator to potential issues or problems.

  21. Participates in Administrative Weekend Rotation.

  22. Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.

  23. Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.

  24. Follows all local, state and federal regulations as they pertain to the position.

  25. Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

  26. Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

Experience and Skills:

This position is full-time offering competitive compensation, a robust benefit package and generous paid time off.

Basic Qualifications

Education/Training/Certifications: A minimum of a bachelor’s degree in social work or a human services field including, but not limited to sociology, special education, rehabilitation counseling or psychology.

Skill(s): Knowledge of computer systems helpful

Experience: One year of supervised social work experience in a health care setting working directly with individuals.
