Job Description:

LIFE Senior Care, a program designed to assist seniors with their customized healthcare needs, located in Jersey City Medical Center, is currently seeking a full-time or part-time Primary Care Geriatrician!

General Responsibilities

Reporting to the Medical Director, the Primary Care Geriatrician performs tasks that include performing in-person comprehensive history and physical on new Lutheran Senior Healthcare participants and coordinating with the interdisciplinary team to develop a comprehensive care plan for each participant.

Essential Duties

  • Supports the mission and values of Lutheran Social Ministries of New Jersey, which is: Through the power of the Holy Spirit, and in response to God’s love as revealed in the Gospel, the mission of the Lutheran Social Ministries of New Jersey is to serve those in New Jersey who hurt, are in need, or have limited choices.
  • Performs in person comprehensive history and physical on new Lutheran Senior Healthcare participants. Coordinates with the interdisciplinary team to develop a comprehensive care plan for each participant.
  • Works collaboratively with the Nurse Practitioner and entire interdisciplinary team.
  • Conducts an in person reassessment semi annually and as needed.
  • Integrates the primary care treatment plan into the overall plan of care developed by the interdisciplinary team. Interacts with team members to meet emergent and acute need of participants. Participates in discharge planning for acute and long-term placement.
  • Evaluates and treats participants during acute illness.
  • Oversees and manages participant’s use of medical specialist and inpatient care.
  • Admits participant to the hospital. Provides primary care (attending) responsibilities for medical management; provides updates to Interdisciplinary team. In conjunction with the Interdisciplinary team plans and coordinates discharge.
  • Refers participants to medical specialist as indicated, communicates and follows up with specialist regarding outcome of visit.
  • Manages all care of participants in the nursing home: Provides regular visits as dictated by nursing home standards and participant need. Performs telephone contact with nursing home staff as required; admits nursing home participant to hospital when necessary.
  • Participates in rotating night and weekend call.
  • Functions as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24 hour care delivery
  • At the direction of the Medical Director reviews medical outcomes/statistical and utilization data, including reviewing of necessity of hospitalizations, and use of specialists. When problems are identified he/she works with the Medical Director to establish best practices and works with clinical staff and interdisciplinary team to make appropriate procedural and or operational changes to avoid unnecessary hospitalizations and improve the health of participants.
  • Completion and documentation of appropriate diagnostic coding.
  • Advises the Medical Director in ways and means to establish better accountability of Lutheran Senior Healthcare services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
  • Assists with the development of policies and procedure, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Follows all Lutheran Senior Healthcare Policies and Procedures and OSHA safety guidelines.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional licensure and certifications.
  • Provides information about Lutheran Senior Healthcare to interested individuals and groups in adherence to PACE regulations.
  • Participates in and supports Quality Improvement initiatives.
  • Performs other duties as required or requested.

  • Provides valuable improvements to participants and Lutheran Senior Healthcare through the review and analysis of utilization data.
  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.
  • Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
  • Follows all local, state and federal regulations as they pertain to the position.
  • Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
  • Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

Experience and Skills:

Basic Qualifications:


  1. Doctorate’s Degree (including PhD’s): MD or DO
  2. Current NJ plenary physician license
  3. Board certified - Internal Medicine or Family Practice
  4. NJ CDS Certification and DEA Registration
  5. Board certified - Geriatrics
  6. Current driver’s license and proof of auto insurance


  1. Thorough knowledge of current concepts, theories and practices related to home and community-based care for the elderly and disabled adults.
  2. Working knowledge of health care delivery and financing systems including Medicaid, Medicare, waiver programs, prospective and capitated systems, public health programs and HMO/Managed Care.
  3. Thorough knowledge of physical, mental and social needs of frail older adults.
  4. Effective oral and written communication skills
  5. Ability to lead and work within the interdisciplinary setting
  6. Strong organizational skills.
  7. Able to manage changing priorities per needs of the PACE program and the agency
  8. Legible handwriting.
  9. Working knowledge of PACE regulations
  10. Solid computer skills with mid- to strong back in Microsoft Word, Excel, Outlook and PowerPoint.


Minimum of 2 years working with the frail or elderly

Medical Clearance:

Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
