 LIFE Senior Care, a program designed to assist seniors with their customized healthcare needs, located in Jersey City Medical Center, is currently seeking Certified Home Health Aides!

This position is per diem (as needed) offering flexible scheduling and competitive pay!’

 

Under the direction and supervision of the Home Care Coordinator/ Registered Nurse, assists participants in the home with duties essential to the patient’s health in accordance with the patient’s individualized care plan.

  • Consults each day with the Home Care Coordinator/Registered Nurse to be update on Lutheran Senior Healthcare participant’s status changes in care plan and to confirm scheduled visits.

  • Under the direction and supervision of the Home Care Coordinator/ Registered Nurse, assists participants in the home with the following duties :

  • Assist patients with personal care needs such as bathing, elimination needs, transferring, dressing / undressing, and ambulation together with any household task (e.g., making bed, changing linens, laundry, light housekeeping, etc.) essential to the patient’s health in accordance with the patient’s individualized care plan.

  • Plan / prepare nutritious meals and assist in feeding patients as directed.

  • Document services provided in an accurate and timely manner in accordance with agency policy and personal care plan. Maintain participant confidentiality at all times.

  • Measure and record patient’s temperature, pulse, respiration and blood pressure when requested and / or directed by the Homecare Coordinator and participant care plan.

  • Report any changes in the patient’s condition or incidents to Nursing Department in a timely manner.

  • Performs tasks / skills as outlined on care plan competently

  • Notifies Nursing Department when clarification or education of a task is required.

  • Submits timecards / activity sheets in a timely manner (i.e. weekly)

  • Implements appropriate infection control practices as per agency policy..

  • Demonstrates appropriate judgment skills to be able to make independent decisions in routine participant care matters.

  • Assist participant with Passive Range of Motion (PROM) when requested and / or directed by the Homecare Coordinator/Physical Therapist or per the plan of care.

  • Maintains safe working environment.  Follows Lutheran Senior Healthcare Safety Policies and Procedures.

  • Participates in continuing education classes and any required staff and training meetings.  Maintains professional affiliations and any required certifications.

  • Participates in and supports Quality Improvement initiatives.

  •  Initiates suggestions for process improvement and/or cost saving measures that enhance care.

  • Uses excellent interpersonal communication skills and is seen as a role model in

  • dealing with different populations.

  •  Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

  • Demonstrates a clear understanding of the Mission, Purpose and Philosophy.

  • Completes annual compliance and HIPAA training and exhibits behavior as set forth by the LSMNJ Code of Ethics and Business Integrity in the performance of duties.

  • Adheres to the Organization’s Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.

  • Follows all local, state and federal regulations as they pertain to the position.

  • Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.

  • Other duties as assigned by my supervisor, necessary for the efficient operation of the department/facility.

     

Experience and Skills:

Basic Qualifications:

 

Education/Training/Certifications:

  1. High school diploma or GED

  2. Certified Home Health Aide License --New Jersey

  3. Current driver’s license with proof of auto insurance

  4. Safe driving record

     

    Skill(s):

  1. Knowledge of principles, practices, standards and techniques of a home health aide

  2. Knowledge of common safety hazards and precautions to establish a safe work environment

  3. Knowledge of medical equipment use and maintenance

  4. Skilled in establishing and maintain effective working relationships with patients, co-workers, medical staff and the public.

  5. Skilled in identifying and recommending problem resolution.

  6. Ability to communicate clearly and effectively.

  7. Ability to react calmly and effectively in emergency situations.

  8. Ability to work independently with minimum supervision

     

    Experience:

    Minimum of 1 year working as a CHHA with the frail or elderly

     

    Medical Clearance:

    Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
